 HICA NEW MEMBERSHIP FORM

There are several ways to complete and submit the form.

(1) Open the Form, Fill it out Electronically, then Print it. Once the form is printed,
sign it on the 2nd page and send it to HICA via postal mail or by hand delivery to our Hilo Office.

(2) Print out the form while it is blank, hand-fill out the entire form and send it to HICA via postal mail or by hand delivery to our Hilo Office.

(3) You may also Fax the Completed Form to HICA. You will need to come into our office to physically sign the application and complete your Member Dues Payment.

Once we receive your completed form, we will contact you regarding the status of your application and to arrange for payment of Member Dues.
